Sometimes with Guru you need to exit to main menu and log back in on your character. I got the prompt to visit him every time except for the final cap raise.

I spent 3+ years trying to figure out how to unlock this achievement and this was all I had to do. lol

Although Goku, Vegeta, Vegito and SSGSS Gogeta also use this form, they only transform in cutscenes and their playable versions are pretransformed, and thus don't actually get the benefit of the transformation.
