Medical Certification Forms MCSA-5875 & MCSA-5876

The date found on the top right corner of the Medical Examination Report Form, MCSA-5875 and Medical Examiner’s Certificate, MCSA-5876, is the date of expiration for the Office of Management and Budget (OMB) approved information collection 2126-0006, that includes the Medical Examination Report Form, MCSA-5875 and Medical Examiner’s Certificate, MCSA-5876

Previously the expiration date was 11/30/2021.
Previously the expiration date was 09/30/2019.
Previously the expiration date was 08/31/2018.

Expiration Date on Driver Examination Forms
Information from NRCME Support:
The current forms posted on the FMCSA website are scheduled to expire on August 31, 2018. However, FMCSA has submitted the information collection renewal request for approval. Since the information collection renewal request has been submitted, we have approval to continue use of the forms that are currently posted on the FMCSA website until we receive notification from OMB. Once the information collection renewal has been approved, new versions of the Medical Examination Report Form, MCSA-5875 and Medical Examiner’s Certificate, MCSA-5876 will be posted on the FMCSA website indicating that the collection has been renewed.What does this mean to Medical Examiners and Drivers?

  • Continue using the forms with the expiration date of August 31st, (Form MCSA-5875 & Form MCSA-5876) until FMCSA publishes new forms.

Extension of Compliance Dates for the Medical Examiner’s Certification Integration Final Rule — see FMCSA official website explanation.
The interim final rule has been extended through June 21, 2021.
What does this mean to Drivers and Medical Examiners?

  • Continue using the original paper Medical Examiner’s Certificate, Form MCSA-5876 for all qualified drivers including Commercial Learner’s Permit/Commercial Driver’s License applicants/holders.
  • Continue submitting paper copies of Medical Examiner’s Certificates, Form MCSA-5876 to State Driver’s Licensing Agencies.

The Physical Qualifications Have Not Changed

The physical qualifications (FMCSA regulations and guidelines) to be applied by a medical examiner to qualify drivers for a medical card have not changed, even though the forms have changed since 2015.

The differences you’ll notice:

  • The forms ask for more information than the previous forms, used before the changes in 2015.
  • Medical examiners are being required to submit more information to support their qualifying decisions, especially where medical conditions exist. This means medical examiners are being more thorough than some used to be, regarding existing medical conditions.

What this means for drivers:

  1. Be well prepared before you get to your DOT physical appointment.
  2. Choose your medical examiner wisely!
  • Find a DOT physical doctor who understands the big picture of commercial driving, and the lifestyle challenges you face.
  • You want to be evaluated by an medical examiner who considers your overall health condition and fitness to drive.
  • Just going to any examiner who may disqualify you on a checklist item on the form, may cost you lost time, lost money, or even your job.

How to submit a Medical Certificate to the Medical Certification Unit

Federal regulations require that every commercial learner permit (CLP) holder or driver license (CDL) holder certify to the type of commercial driving they currently perform (or expect to perform).

  • these requirements affect all drivers who have or apply for a commercial driver license (CDL)
  • a USDOT Medical Examiner's Certificate will be required for some types of driving and must be submitted to DMV before your current certification expires
  • you will lose your commercial driving privileges if you do not comply

The DMV will send CDL/CLP holders a "Notice of Non-Compliance and License Downgrade" if their medical certificate on file expires resulting in their medical certification status becoming "Not Certified." CDL holders will be downgraded to a non-CDL class if an updated Medical Examiners Certificate (MCSA-5876) is not submitted and added to the drivers record within 55 days of the letter date.  CLP holders will have their permits cancelled if an updated Medical Examiners Certificate (MCSA-5876) is not submitted and added to the drivers record within 55 days of the letter date. Employers that subscribe to the NY State License Event Notification Service (LENS) can sign up to receive notification of these status changes. Passenger carriers enrolled in the 19-A system will also receive status change notifications.


Certification and driving types

If you have a commercial driver license, Class A, B or C or CDL permit issued by the NY State DMV you must certify the type of driving you do anytime you perform a license transaction (other than an address change, or duplicate license). You will be required to choose only one driving type from the four federal driving types listed below.

You must also keep a current USDOT Medical Examiners Certificate on file with DMV if you self-certify to a Non-Excepted Driving type (NA or NI).


The four federal driving types

Non-Excepted Interstate (NI)

Commercial driving operation is permitted across state lines, and you are required to meet the Federal medical requirements.

  • you must be age 21 or older
  • if you have a HAZMAT (H) or HAZMAT/Tank (X) endorsement, you may select this driving type
  • you must not have a K (CDL Intrastate Only) or A3 (Med Cert Exempt) restriction on your driver license

Non-Excepted Intrastate (NA)

Commercial driving operation is restricted to New York State only, and you are required to meet the Federal medical requirements.

  • you must be age 18 or older
  • drivers with a HAZMAT (H) or HAZMAT/Tank (X) endorsement may select this driving type
  • you must have a K (CDL Intrastate Only) restriction on driver license
  • you must not have an A3 (Med Cert Exempt) restriction on your driver license

Excepted Interstate (EI)

Commercial driving operation is permitted across state lines for excepted operation only and you are exempt from the Federal medical requirements.

  • you must be age 18 or older (21 or older when noted for certain excepted operations)
  • you must have an A3 (Med Cert Exempt) restriction on your driver license
  • you must not have a K (CDL Intrastate Only) restriction on your driver license

Excepted Intrastate (EA)

Commercial driving operation is restricted to New York State, permitted for excepted operation only and you are exempt from the Federal medical requirements.

  • you must be age 18 or older (21 or older when noted for certain excepted operations)
  • you must have the K (CDL Intrastate Only) and the A3 (Med Cert Exempt) restriction on your driver license

Note: If you cannot meet the criteria for one of the driving types defined above, you can no longer hold a CDL. You must go to a DMV office and downgrade to a non-commercial class driver license.


How to obtain a USDOT Medical Examiners Certificate

You must get a USDOT physical performed by a medical examiner listed on the Federal Motor Carrier Safety Administration’s (FMCSA) National Registry. To find an approved medical examiner, you can either go to the National Registry of Certified Medical Examiner Search, or call FMCSA at (202) 366-4001 and ask to speak to a representative. Once your physical is complete, the medical examiner will give you a Medical Examiners Certificate (Form MCSA-5876). To avoid a downgrade or cancellation of your CDL/CLP, you must take this certificate to the DMV when you apply for your CLP/CDL, or fax/email it to the NY State DMV Medical Certification Unit. (See important reminders below.)


How to confirm that the Medical Certificate is on my driving record

DMV will send notifications regarding CDL/CLP downgrade/cancellation events to passenger carriers enrolled in the 19-A system. Drivers can also monitor their own CLP/CDL medical certification status through MyDMV and subscribe for text and/or email notifications.


Drivers with a HAZMAT (H) or Hazmat/Tank (X) endorsement

You are only eligible for the HazMat or Hazmat/Tank Endorsement if you are age 21 or older and you meet the Federal medical requirements.


Medical variances and "V" restriction

If your USDOT Medical Examiner's Certificate indicates "must be accompanied by a Diabetic, Hearing, Seizure or Vision waiver exemption or an SPE Certificate," you must have a Medical Variance issued by FMCSA.


Important reminders

  • Verify that your address is current on DMV records. 
  • If you select NA or NI as your driving type, you must send in a photocopy of your USDOT Medical Examiner's Certificate to the Medical Certification Unit, prior to the expiration date of the USDOT Medical Examiners Certificate on file with DMV.
  • Photocopied USDOT Medical Examiner's Certificate must be legible, complete and on one side of a standard 8½" x 11" sheet of paper
  • If you have a Medical Variance, you must keep it current with the FMCSA.
  • The DMV will not remind you when your USDOT Medical Examiner's Certificate or Medical Variance is about to expire unless you are enrolled in the Electronic Reminders program.

If you fail to maintain current, updated information with the DMV, you will lose your commercial driving privileges.
