AHA Coding Clinic® for ICD-10-CM and ICD-10-PCS - 2018 Issue 3; Ask the Editor

A patient is admitted for treatment of chronic atrial fibrillation (AF) with rapid ventricular response (RVR). The classification provides codes that describe paroxysmal, chronic, and persistent atrial fibrillation, but not atrial fibrillation with rapid ventricular response. A diagnosis of chronic AF with RVR seems to indicate severity. Would it be appropriate to assign code I48.0, Paroxysmal atrial fibrillation, for AF with RVR? If not, what code should be assigned to reflect chronic atrial fibrillation with RVR? ...

Hi,

I'm trying to figure out how to code Atrial Fibrillation with Rapid Ventricular Response in ICD-10. What would the code be for RVR? Or should I just code the unspecified atrial fib only - I48.91?

Thanks,
Kim

How do you code atrial fibrillation with rapid ventricular response?

Answer: You should report 427.31 (Atrial fibrillation) for this diagnosis, which you may see documented as Afib with RVR. Codes 427.41 (Ventricular fibrillation) and 427.42 (Ventricular flutter) are specific to fibrillation and flutter respectively, so you should not use those codes without supporting documentation.

What is atrial flutter with RVR?

Atrial fibrillation with rapid ventricular response is a fancy name for an irregular heartbeat. When your heart's electrical signals aren't working right, it can lead to a heartbeat that's too fast. This abnormal heart rhythm is what doctors call atrial fibrillation, or AFib for short.

What causes atrial flutter with rapid ventricular response?

Rapid ventricular rate or response (RVR) AFib is caused by abnormal electrical impulses in the atria, which are the upper chambers of the heart. These chambers fibrillate, or quiver, rapidly. The result is a rapid and irregular pumping of blood through the heart.

Is a flutter a ventricular response?

Atrial flutter is a relatively common supraventricular arrhythmia characterized by rapid, regular atrial depolarizations at a characteristic rate around 300 beats/min and a regular ventricular rate corresponding to one-half or one-quarter of the atrial rate (150 or 75 beats/minute).
